WellCheck joins St. Mary's 360 to expand connected care across St. Mary's County
The St. Mary's County Health Department has been awarded $3,530,907 through the Maryland Rural Health Transformation Program to launch St. Mary's 360: Connected Care Everywhere. WellCheck is a project partner supporting community-integrated care coordination and digital health literacy across the county.
WellCheck, a Maryland-based public health technology company, has been named a project partner in St. Mary's 360: Connected Care Everywhere, a countywide initiative led by the St. Mary's County Health Department.
The Health Department received $3,530,907 through the Maryland Rural Health Transformation Program to launch the initiative, which is designed to improve access to care and health outcomes for county residents. St. Mary's 360 brings together local organizations, trusted services, and technology so residents can connect with healthcare, behavioral health, oral health, wellness programs, digital health support, and other community resources.
“WellCheck is proud to continue working alongside the St. Mary's County Health Department and its community partners to make care easier to find, access, and navigate. Technology is most effective when people have the knowledge and confidence to use it. By supporting digital health literacy and creating clearer connections among residents, care teams, and community services, St. Mary's 360 can help more people take an active role in their health and receive the support they need.”
Chris Nickerson, Chief Executive Officer, WellCheckWhat the initiative will do
Under St. Mary's 360, the county's Health & Wellness Portal will expand as a local directory, referral, and care-coordination resource. The broader initiative also includes:
- Community-integrated care coordination. Dedicated care coordinators will work across clinics, schools, community programs, and other local access points, using connected digital tools to help residents navigate services, complete referrals, and stay engaged with ongoing care and support.
- Assistance with telehealth, remote monitoring tools, and eligible home health-monitoring devices.
- Mobile, home-based, and community-based outreach for residents who experience barriers to care.
- Stronger connections to primary care, chronic-disease prevention, behavioral health, oral health, and wellness services.
- Improved coordination among healthcare providers, public health programs, and community organizations.
“People should not have to figure out the health system alone. St. Mary's 360 brings partners, technology, and trusted local services together so residents can get connected and stay connected—to health care, to wellness support services, and to better health.”
Dr. Meena Brewster, Health Officer, St. Mary's CountyThe initiative builds on existing services and partnerships to create a more connected local system, one that reaches residents earlier, supports them longer, and helps them navigate the path to better health.
Community-integrated care coordination
A central component of St. Mary's 360 is community-integrated care coordination, which brings care coordination into the clinics, schools, community programs, and other trusted settings where residents already live, learn, work, and receive care. Supported by WellCheck's digital infrastructure, this approach helps residents navigate services, complete referrals, and remain connected to ongoing care.
Coordination happens where people already are
Care coordinators work from the settings residents already trust rather than from a single central office. A referral can start at a school, a clinic visit, a community program, or a resident's own request through the Health & Wellness Portal.
Referrals are tracked to a documented outcome
EquiLoop tracks a need from screening through documented resolution, so a coordinator can see whether a resident actually reached the service rather than only whether a referral was sent.
Digital health literacy is part of the work
The initiative helps residents build the skills and confidence to use the Health & Wellness Portal, telehealth services, remote-monitoring tools, and other digital health resources. Tools only help people who can use them.
Partners share a coordination path
Health care providers, public health programs, and community organizations often serve the same residents without a shared intake path. Connected coordination gives partners one place to see what has been requested, what has been delivered, and what is still open.
Project partners and supporting organizations
